Pride Health is seeking Licensed Practical/Vocational Nurses (LPN/LVN) to join the Home Health team in Greensboro, NC . In this role, you’ll provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN). You’ll deliver compassionate, high-quality care while ensuring compliance with state, federal, and Medicare standards.

Key Responsibilities:

• Deliver skilled nursing care per physician orders and RN supervision.
• Identify patient physical, psychosocial, and environmental needs; report outcomes to RN Case Manager.
• Participate in care planning and coordination with interdisciplinary teams.
• Educate patients/families on self-care techniques and assess effectiveness.
• Maintain accurate documentation in Homecare Homebase (HCHB) .
• Ensure infection control and safe clinical practices (OSHA standards).
• Assist RN/physician with specialized duties, including end-of-life/hospice care.
• Support admitting, transferring, and discharging patients.
• Participate in on-call duties (one weekday per week).
• Travel to patient homes across Randolph, Guilford, Alamance, and Rockham counties .

Required Skills & Competencies:

• Strong adaptability and problem-solving skills in home health settings.
• Knowledge of infection control procedures and home health nursing standards.
• Proficiency with clinical equipment and electronic documentation.
• Ability to work independently and manage time efficiently.

Qualifications:

• Graduate of an accredited Licensed Practical/Vocational Nursing program.
• Active LPN/LVN license in North Carolina (or compact state).
• 2+ years’ experience in a clinical setting (Home Health or Hospice preferred).
• Proficient in Homecare Homebase (HCHB) , OASIS , and Home Health workflows.
• Current CPR certification .
• Valid driver’s license and reliable transportation with auto insurance.

Location: Greensboro, NC 27408

Job Type: 3 months Contract

Schedule: Monday – Friday | 8:30 AM – 5:00 PM | 40 hrs/week

Travel: Randolph, Guilford, Alamance, and Rockham counties.

Local Pay range: $30-$33/hour

*Offered pay rate will be based on education, experience, and healthcare credentials.

Travel Gross Pay: $1500 - $1640/Week
